It's to do with displacement of the head and or body, check the passfiles or if you have a Hook5 Customisable skin press F4 open the skin with Hook GUI and check the displacement of each body part

its caused by the seams on the normals,virtually every skin has this problem

Yes attach the toy in free mode then attach the hand to the toy,obviously you'll have to move the toy to the hand in the first place

then when you move the toy the models hand will stay attached to it

make sure texture tracking is disabled in Hook settings

Hook5 customizable skins should always be set at 1x afaik,mine are anyway,all work correctly even with 4x mod installed,for correct placement of in game nipples
