The addon goes in the addon folder (The folder with scene, script, and image folder in it)
The texture goes in mod/activemod (Usually a folder named after the toy with a bunch of images in it and no folders)

Usually you would sculpt that in to the body mod. A high poly body mod would handle it better though. The other thing that works fairly well is adjusting the but rotation down for a tighter crease.
